A mixture of heroin and alcohol has been confirmed as the cause of death of Glee star Cory Monteith.
Canadian authorities have confirmed the lethal combination according to gossip website TMZ.com and Reuters
The British Columbia Coroners Service said in a statement that an autopsy and toxicological analysis showed the 31-year-old actor died on from a "mixed drug toxicity, involving heroin and alcohol".
However, it said it was unlikely the death was intentional.
"It should be noted that at this point there is no evidence to suggest Mr Monteith's death was anything other than a most-tragic accident," the Coroners Service said.
Cory was discovered dead in his hotel room at the Fairmont Pacific Rim hotel in Vancouver, Canada on Saturday July 13 (Sunday NZT). He was found by hotel staff after he failed to check out at the designated time.
The Canadian actor, who played Finn Hudson in musical drama show Glee, voluntarily checked into rehab for a month in March to battle drug addiction and was released in April.
He had spoken publicly in the past about his struggle with drug addiction, which began in his teens.
